Sarah Mitchell-Gears
Sarah is the Principal Pharmacist for Older People’s Mental Health at Bradford District Care NHS Foundation Trust (BDCFT), with 35 years’ experience across all sectors of pharmacy. She leads the delivery of consultant-level pharmaceutical care across Bradford District and Craven, working closely with multidisciplinary teams, care homes, primary care, carers, and inpatient services to optimise medicines use and improve outcomes for older adults with severe mental illness and dementia.
Prior to joining BDCFT, Sarah spent 20 years in secondary care, predominantly working with older people. She holds a Postgraduate Certificate in Mental Health Therapeutics from Aston University and has completed a National Institute for Health and Care Research (NIHR) Clinical Research Internship.
Passionate about medicines optimisation and patient safety, Sarah has led a pioneering project exploring the deprescribing of antipsychotics for behavioural and psychological symptoms of dementia (BPSD) in care homes. This work has improved understanding of the barriers and facilitators to deprescribing and informed local policy and practice recommendations to support safer, more appropriate prescribing in this vulnerable patient group.
A strong advocate for medicines safety, Sarah chairs the West Yorkshire Integrated Care System Anticholinergic Burden (ACB) Task and Finish Group. She has led the development of clinician guidance, patient information resources, educational initiatives, and regional programmes to reduce the harms associated with anticholinergic medicines. She has numerous publications, has contributed to webinars and podcasts, and is the author of the British Geriatrics Society blog, A Call to Review: Are Your Patients Carrying an Anticholinergic Burden?
She is committed to supporting colleagues, developing future pharmacy leaders, and advancing the role of pharmacy in improving patient care and outcomes.
