UK MS Register COVID-19 case report form
On behalf of UK MS Register we would like to ask you to report any cases of MS patients with COVID-19 you have come across.
UK MS Register have created a secure, anonymised case report form (CRF) for healthcare professionals to report confirmed infections in patients with MS. It’s a quick survey and an opportunity to collect UK data which can tell us how vulnerable patients with MS are to the virus, and what is the clinical outcome of the infection, in real time. This can guide our next wave of COVID-19 guidelines.
The CRF does not collect patient identifiable information, but requires the reporter's contact details.

Perhaps, in any telephone consultations and consider adding a supporting statement in your letters encouraging patients to register to the study.
